Registration Instructions

We encourage you to register early. Space is limited in the workshops.

Conference registration forms can be filled out and submitted in 3 ways:
1. Online Submission (preferred) with Payment Mailed
  • Use the online form links, fill out the forms online and submit forms.
  • Send payment check as detailed in Payment and Mailing Instructions section below.
2. Email Submission with Payment Mailed
  • Print the soft copy forms and fill them out. PLEASE PRINT LEGIBLY.
  • Scan the filled out forms to PDFs.
  • Send PDFs of completed forms via email to mvbt.bwa@gmail.com
  • Send payment check as detailed in Payment and Mailing Instructions section below.
3. Hard Copy Submission with Forms and Payment Mailed
  • Print the soft copy forms and fill them out. PLEASE PRINT LEGIBLY.
  • Send the completed hard copy forms via mail as detailed in Payment and Mailing Instructions section below.
  • Send payment check as detailed in Payment and Mailing Instructions section below.

Form G: Group Registration Form
Registrants who are part of a group/chapter should use this form. One of these forms should be submitted for each person in the group.

The group contact MUST also use this form to provide summary counts for the group and get a total for the group's conference cost.
